2021 Fall Session

September 27 – December 18 (no lessons November 22-26)

All participant applications must be completed annually. All other forms are to be completed by the participant or the parent/guardian of those who have received registration information from the program director.

LTR, Inc. is a PATH Intl. Premier Accredited Center and operates under the guidelines and standards established by our accrediting organization.  All paperwork is required and must be complete before participation in any LTR program. 

COVID-19 Policy

September 1, 2021

Beginning September 1, all volunteers, staff, and participants will be required to wear masks regardless of vaccination status when inside the barn. Participants are permitted to remove masks once they are mounted. All participants must sign a COVID-19 Waiver; you may either bring the waiver with you, or scan and email back to kblaine@ltrf.org.

Teacher/Therapist Assessment Form (Participants receiving physical therapy/occupational therapy/speech-language therapy may print the Therapist Assessment Form to provide additional information regarding current therapies.)
